A black and white photo means that no colors other than shades of black and white appear in the photo. It does not mean that the photo only consists of black or white pixels, normally there is a huge variety of grey. Black and white photos are either done by using black and white film or by reducing the colors of a colored photo when digitally processing it down to grey colors.
